Transition from childhood to adulthood


Transition from children to adulthood can be fraught with uncertainty for some young people with additional needs and their families. Our transition service is to help all disabled young people from the age of 13 to 25.

We will help you plan key changes at age 16, 18 and if needed, beyond 19. We make sure that a young person with an additional need and their carers have access to a planned, positive progression for when they approach adulthood.

Professionals involved during transition include social workers, community nurses (learning disabilities) and continuing health care nurses.
