Learning Disabilities Service

Who we are and what we do

We have 5 Learning Disability Teams in the County. The Teams are made up of Social care staff and Health Service staff.

What are Integrated Learning Disability Teams?

The Integrated Learning Disability Teams are made up of staff from Social Care and the Health Service who work together in each area. By working together it is hoped to provide a better service to people with Learning Disabilities.

Discussion

Once you get in touch with Social Care and Health someone from an Integrated Learning Disability Team will carry out an assessment. This means finding out what you need. Then services will be found to meet these needs. Sometimes a member of the team may provide a service, such as speech therapy. Other services, like day care, will be arranged with a care provider.

A member of the team called a care co-ordinator will make sure the services are meeting needs properly. Reviews will be carried out to make sure things are going well. Services can be changed if necessary.

Why have integrated teams?

You will only get one assessment - all information will be shared within the team. You will only need to phone one number at Social Care Direct.
