XS - Young Persons' Substance Misuse Service
XS is a multi-agency service to provide help and treatment to young people under 19 who have difficulties with drugs and/or alcohol.
XS positively welcome enquiries, and professionals can seek advice, information and support whilst maintaining the anonymity of a young person. The service is for young people who have a serious drug or alcohol problem and who are vulnerabe, for example they may be children in need, children looked after, at risk of or offending.
XS has two teams across County Durham, covering the north and south.
XS will:
- Provide a specialist assessment and care plan
- Provide access to a range of services including harm minimisation, 1-1 therapeutic support, prescribing services, outreach support, family support, drug education, motivational interviewing, relapse prevention, life skills and diversionary activity
- Co-ordinate the provision of care provided by other agencies and professionals
- make referrals to external sources of support where necessary
- Meet with the young person once per week as a minimum wherever possible during the treatment process
- Contribute to the care already provided by statutory agencies working as a provision on existing care plans where necessary and appropriate
