County Durham transition standards for young people with learning difficulties and/or disabilities from age 13 onwards. A reference document for school staff, Connexions and Children and Young People’s Services staff. This set of quality standards is designed for use by all professionals involved in transition planning. The standards have been produced in partnership between Connexions, Children and Young People’s Service, Adult and Community Services, and Tees, Esk and Wear Valleys NHS Trust (formerly County Durham and Darlington Priority Services NHS Trust).
